Title: Surgical Strategies Protecting against Right Ventricular Dilatation following Tetralogy of Fallot Repair Authors: Amr Arafat ([email protected]) Elatafy Elatafy ([email protected]) Sahar Elshedoudy ([email protected]) Mahmoud Zalat ([email protected]) Neamat Abdallah ([email protected]) AHMED ELMAHROUK ([email protected]; [email protected]; [email protected]) Version: 1 Date: 23 Dec 2017 Author’s response to reviews: Reviewer #1: This retrospective review of patients who underwent a valve sparing TOF repair is consistent with the many publications in this area in the literature. It supports the general idea, but does not explore the limitations of the technique, specifically, how small a valve is too small and how much residual RVOTO is acceptable? Thank you very much for those valuable comments. We selected our patients according to their preoperative pulmonary annulus z score which ranged from -2 to -3. In our experience and others preservation of the valve in patients with z-score less than -3 is associated with recurrent RVOTO. We highlighted this in the discussion in the following paragraph.

Main indications and long-term outcomes of reoperation after initial repair of tetralogy of Fallot
Background and Objective: The aim of this study was to analyze our indications, surgical procedures, and clinical outcomes of patients undergoing reoperation after surgical correction of tetralogy of Fallot (TOF). Methods: Thirty seven consecutive patients who underwent reoperation late after intra-cardiac repair of TOF within a period of 10 years were assessed. Results: The most co...
متن کاملبررسی عملکرد بطنها با اکوکاردیوگرافی در نارسایی دریچه ریوی پس از جراحی تترالوژی فالوت
Background: Tetralogy of Fallot is the most common cyanotic congenital heart disease. The systolic and diastolic function in both ventricles is altered even after successful corrective surgery for this defect with a transannular patch. Pulmonary regurgitation, a common complication after this treatment, is usually well tolerated in childhood. The aim of this study was to assess the combined dia...
متن کاملSurgical strategies protecting against right ventricular dilatation following tetralogy of Fallot repair
BACKGROUND Right ventricular (RV) volume overload increases morbidity and mortality after tetralogy of Fallot (TOF) repair. Surgical strategies like pulmonary leaflets sparing and tricuspid valve repair at time of primary repair may decrease RV overload. Our objective is to evaluate early and midterm results of pulmonary leaflets sparing with infundibular preservation and tricuspid valve repair...
متن کاملRight ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease
Introduction: In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...
متن کاملAlterations in regional right ventricular shape in patients following tetralogy of Fallot repair involving a transannular Patch
Background Patients who undergo surgical repair of Tetralogy of Fallot (TOF) often experience abnormal remodeling of the right ventricle (RV), notably with chamber dilatation and associated dysfunction. The ideal surgical technique is controversial, but RV outflow tract repair utilizing a transannular patch (TAP) is employed to overcome the RV outflow tract obstruction in many patients with TOF...
